You asked for it!!

Quite a few of you have asked me what a care plan is and what it looks like. So here goes...a comprehensive care plan to bore you to death on a Friday... ha ha. No, honestly, I do learn a lot from them! As much as I hate them, I actually like them, once it all starts coming together. The pics are a bit hard to see...you can come over anytime you like and see it...yeah right!! ha ha



This is our mapping page. You have to patho the disease process and how it affects the body. You have to write two Nursing diagnosis' along with signs and symptoms, goals, interventions and evaluations. This map is one of my smaller ones at 10.5 pages. Here is a close up of my patho. It was for septic shock...so fun!


Then the pic below is of 6 different assignments that go with the plan. I had a 14 page in depth assessment of my patient and why each intervention is done, a 6 page med sheet where you have to know everything about the meds you are giving, a 3 page lab form where you have to analyze why each lab is either elevated or decreased, a full psychosocial assessment about the patient and their family dynamics, an in depth page about the A-line, swan gans, quinton caths, and any other tube the patient has placed, and finally a reference page to site all the books we used.


So now that I have bored you to death, have a great weekend! I sure will be sleeping good, however, I have a ton of studying to do. A test in two weeks again! Cannot wait until May!!!
